XERF Skin Tightening: Now Available in San Diego

XERF is the newest name in non-surgical skin tightening, and it's now available at Medizen Medspa's Mira Mesa clinic in San Diego. FDA-cleared in August 2025, XERF is Cynosure Lutronic's first dual-frequency monopolar radiofrequency platform, designed to lift, tighten, and rebuild structural collagen without surgery, needles, or downtime.

What is XERF?

XERF is a non-invasive skin tightening treatment that uses dual-frequency monopolar radiofrequency to remodel skin from the inside out. It's the first device to combine 6.78 MHz and 2 MHz RF energy in a single platform, which lets it target three tissue depths (superficial, mid-dermal, and deep structural) in one treatment, rather than treating just one layer at a time the way older RF devices do.

Practically, that means XERF can address skin laxity, mild jowling, neck looseness, and overall facial structure decline in fewer visits than single-frequency RF platforms.

The system is manufactured by Cynosure Lutronic, the same company behind the Potenza RF microneedling device we already use for collagen induction and the LaseMD Ultra we use for hair restoration. Adding XERF to our menu was a natural extension of platforms we trust.

How XERF works

Dual-frequency monopolar RF

Traditional RF skin tightening devices send a single radiofrequency at one depth. XERF alternates between two frequencies in a single pass:

  • 6.78 MHz: warms superficial and mid-dermal skin to stimulate fibroblasts and trigger collagen remodeling at the layers that drive surface texture and elasticity.
  • 2 MHz: penetrates deeper into the structural fascia, where the lift-and-tighten effect happens.

By delivering both wavelengths in one session, XERF builds collagen at multiple layers simultaneously instead of layering separate treatments over months.

Comfort and cooling

Older monopolar RF treatments (Thermage in particular) were famous for being painful, hot, and requiring vibration distractions or even oral pain medication. XERF was engineered specifically to fix this.

The system uses Wave Fit pulse delivery and Advanced Integrated Cryogen Delivery cooling, which protects the skin surface while the deeper tissue is heated. Most patients describe the sensation as a warm facial massage rather than a treatment that requires gritting through it. Numbing cream isn't required, which is uncommon for RF tightening at this depth.

Real-time skin adaptation

XERF includes Accurate Impedance Feedback, which reads the electrical resistance of your skin in real time and adjusts energy delivery accordingly. With three depth settings and ten intensity levels, the provider can fully tailor each pulse to your skin type, hydration, and the specific area being treated.

What XERF treats

  • Facial laxity: cheek descent, mid-face volume loss, early jowling
  • Jawline definition: softens the marionette-line area, sharpens the lower face
  • Neck and submental laxity: tightens loose skin under the chin and along the neck
  • Brow heaviness: subtle brow lift through forehead tightening
  • Crepey texture: improves crepey skin on the face, neck, and decolletage

XERF is positioned for patients with mild-to-moderate laxity who want lift and tightening but are not yet ready for surgical intervention. It's not a substitute for a surgical facelift on patients with severe laxity, and we'll tell you that during your consultation if surgery would serve you better.

XERF vs other skin tightening treatments

If you've already done research, you've probably seen XERF compared to Ultherapy, Thermage, and Sofwave. Here's how they actually differ:

XERF vs Ultherapy (microfocused ultrasound)

Ultherapy uses focused ultrasound energy to heat tissue at specific depths (1.5 mm, 3.0 mm, 4.5 mm). It's effective but historically uncomfortable, and most patients describe it as one of the more painful aesthetic treatments. XERF achieves similar collagen-stimulating depths with RF instead of ultrasound, and patient comfort is significantly better thanks to the integrated cooling and Wave Fit pulse system.

XERF vs Thermage

Thermage was the original monopolar RF skin tightening device. It uses a single frequency and is famously hot and uncomfortable, often requiring extensive numbing. XERF improves on Thermage in two ways: dual frequencies for multi-depth treatment in one session, and much better cooling for a tolerable patient experience.

XERF vs Sofwave

Sofwave uses synchronous parallel beam (ultrasound) technology focused on a fixed depth (around 1.5 mm). It's excellent for fine line softening and superficial collagen stimulation but doesn't reach the deeper structural tissue. XERF's deeper 2 MHz frequency targets that structural layer, making it a better choice when the goal is meaningful lift rather than texture refinement alone.

XERF vs RF microneedling (Potenza)

These are complementary, not competing. Potenza RF microneedling creates micro-injuries plus RF energy at controlled depths through tiny needles, which is excellent for texture, acne scarring, pore refinement, and dermal-level collagen induction. XERF delivers RF energy non-invasively across larger surface areas at deeper structural levels for lift and tightening. Many patients benefit from both: Potenza for texture and quality, XERF for lift.

XERF compared to our Titanium Facelift

We already offer the Titanium Facelift for non-surgical lifting, which uses Alma Soprano laser energy plus microcurrent. The two services overlap but aren't identical:

  • Titanium Facelift: laser-based, multi-modality, ideal for combined surface refresh and lift
  • XERF: monopolar RF only, deeper structural targeting, fewer total sessions needed for tightening goals

What to expect at your XERF appointment

Before treatment

Arrive with clean skin (no makeup, no oils). Skip retinoids, acids, or aggressive exfoliants for 5 days before your session. Otherwise no special prep is required.

During treatment

A typical full face and neck XERF session runs 45 to 60 minutes. Our provider applies a coupling gel and moves the XERF handpiece in smooth passes across the treatment areas. You'll feel warmth deep in the tissue with the surface staying cool. Most patients describe it as a warm facial massage. No numbing cream is needed.

After treatment

You can return to work, exercise, and normal skincare immediately. Some patients experience mild redness for an hour or two. Avoid heavy heat (saunas, hot yoga) for the rest of the treatment day.

Results timeline

XERF produces both an immediate effect and a progressive effect.

  • Immediate: subtle tightening visible the day of treatment due to immediate collagen contraction.
  • Weeks 4 to 8: noticeable lift and tightening as new collagen production ramps up.
  • Months 3 to 6: peak results as collagen remodeling matures.

Most patients see meaningful results from a single treatment, though many choose to do a series of 2 to 3 sessions spaced 6 to 8 weeks apart for the strongest possible lift. Maintenance treatments once or twice a year preserve results long-term.

XERF cost in San Diego

Pricing for XERF in San Diego typically falls between $1,500 and $3,000 per session depending on treatment area and clinic. A typical full face plus neck session is in the $2,000 range. We'll quote your exact pricing during consultation based on your treatment plan.

Who is a good candidate for XERF?

XERF works best for adults in their late 30s through 60s with mild-to-moderate skin laxity, declining jawline definition, or early neck looseness. It's well-suited for patients who want visible improvement without surgery, downtime, or a dramatically obvious change.

XERF is not recommended for patients who:

  • Have severe, surgical-level laxity (a facelift is more appropriate)
  • Have active skin infections or open wounds in the treatment area
  • Are pregnant or breastfeeding
  • Have a pacemaker, defibrillator, or other implanted electronic device (monopolar RF is contraindicated)
  • Have certain metal implants in the treatment area

Our provider will screen for these during your consultation and recommend a different treatment if XERF isn't appropriate for you.
