Every new Botox patient asks the same question on the way out of their first appointment: how long is this going to last? The honest answer is more nuanced than the marketing brochures suggest. At Medizen Medspa, we'd rather set realistic expectations than over-promise.

Here's exactly how Botox works in your body week-by-week, what affects how long it lasts, and how to get the most out of every appointment.

The honest answer: 3 to 4 months for most patients

Botox typically lasts three to four months in the average treated area for the average patient. Some patients see results closer to four to five months, especially after their second or third treatment as the muscle gradually weakens. A smaller group metabolizes Botox quickly and sees results fade closer to the ten week mark.

If anyone tells you Botox lasts six months on first use, they're either talking about a different product (like Daxxify, which has longer claimed duration) or being optimistic. For traditional onabotulinumtoxinA, three to four months is the real benchmark.

Week-by-week timeline after treatment

Day 1 to 3

You won't see much yet. Botox takes time to bind to nerve endings and block the chemical signal that causes muscle contraction. Some patients notice a slight smoothing within 48 hours, but most see nothing significant.

Day 4 to 7

Results start to emerge. Forehead lines and crow's feet begin to soften. You'll see your most natural-looking expressions starting to flatten.

Day 10 to 14

Full results. By the two-week mark, Botox has reached peak effect. This is when we ask patients to come back for a touch-up if anything needs balancing.

Month 1 to 2

Peak smoothness. Lines stay soft, expressions look refreshed but natural, and most patients feel like the treatment is at its best.

Month 3

Gradual return of movement. You'll start to notice your forehead working more or your crow's feet showing slightly with a full smile. Lines are still softer than baseline.

Month 4

Time to rebook. Movement returns to about 70 to 80% of baseline. This is the sweet spot for the next treatment, before lines fully reappear.

7 factors that affect how long your Botox lasts

  1. Your metabolism. Faster metabolisms break down Botox faster. Genetics plays a real role here.
  2. Dose used. A under-dose wears off faster. We use clinically appropriate dosing tailored to your muscle strength, not bargain-basement under-treatment.
  3. Treatment area. Crow's feet often last longer than forehead lines because the orbicularis oculi muscle is smaller and easier to relax fully.
  4. Muscle strength. Stronger, larger muscles (like the frontalis on someone who expresses heavily with their forehead) need higher doses and may wear off faster.
  5. Exercise intensity. Patients who do heavy cardio multiple times per week often metabolize Botox slightly faster. Studies are mixed but real-world experience supports a small effect.
  6. Treatment consistency. Patients who get Botox regularly for years often find their results last longer over time as the treated muscles weaken.
  7. Product chosen. Daxxify lasts longer (claimed six months). Dysport often takes effect faster but lasts about the same. Jeuveau is comparable to Botox.

How to make your Botox last longer

A few evidence-backed habits can help you stretch results:

  • Wear daily sunscreen. UV breaks down collagen and accelerates the appearance of lines underneath softened muscle activity.
  • Stay consistent with appointments. Don't let the treatment fully wear off between sessions. Coming in at the 3 to 3.5 month mark keeps the muscle weakened and often lets you stretch the next interval slightly.
  • Manage stress. Chronic stress increases muscle tension and facial expressiveness, both of which work against your Botox.
  • Stay hydrated. Hydrated skin maintains a smoother surface that complements the smoothing effect.
  • Skip the rigorous workout day-of. After treatment, avoid heavy exercise for 24 hours to let Botox bind without being flushed by circulation.

What about year-long Botox?

Daxxify (daxibotulinumtoxinA) is the newer FDA-approved neuromodulator with extended duration. In trials, average duration was around six months, with some patients seeing nine months in select areas.
